Forms
Forms allow your camper to participate in activities and to receive medical care in the event of an accident or illness.
All forms must be submitted online through your Camp InTouch account or via the Campanion App by May 1st.
Mailed, emailed or faxed forms will not be accepted.

Required Forms (including medical and transportation forms) can only be accessed by logging in with your username (email address) and password via a web browser or the Campanion App.

Login to your child’s Camp InTouch account. Login Here. Once you are logged in, click “Forms and Documents” to view all of your online forms.
